New Patient Policy (or those who have not been seen for over 3 years):
We ask that you fill out new patient paperwork prior to scheduling an appointment. We have 3 options for your convenience. The quickest way to accomplish this is to complete the secure online form. This will be sent to a secure email and requires no further follow up from you. Once we receive the paperwork, we will be aware that you would like an appointment and will call you to schedule. The second option is to print out the forms and mail to us at PO Box 515, Smyrna, DE 19977. Once received, we will call you for an appointment. The 3rd option is to pick up the paperwork in the office at 28 Deak Drive or to have us mail it to you. Please be advised that this take considerably more time than the previous options but is an option to you if desired. We have found that by implementing this policy it has allowed us to spend the majority of your visit focused on the problem you are experiencing rather than the necessary paperwork.
